We are currently accepting Medicaid plans for outpatient therapy and ADHD evaluations. For more information on using Medicaid for psychological testing, please contact our office!

HELPFUL TIPS TO DETERMINE YOUR COVERAGE

Ask your insurance provider:

  • Do I have mental health benefits for outpatient therapy / psychological testing?​

  • What amount is my deductible (if applicable) and has it been met? What is the cost of services before and after my deductible is met? ​

  • Do I need pre-approval for services?

  • Is approval required from my primary care physician for testing?

  • Is Five Lakes Counseling & Psychological Services considered in-network?

  • Is there a limit to how many sessions (for therapy) or hours (for testing) my benefits will cover?

If you are covered by more than one insurance plan, contact your insurance providers to determine which plan is primary, secondary, tertiary, etc. Understanding your coordination of benefits will ensure proper and timely billing of insurance for services.​

NO INSURANCE? MENTAL HEALTH SERVICES NOT COVERED?

Clients who do not have mental health coverage may schedule at our clinicians' direct pay rates. Contact our office to receive a Good Faith Estimate for direct pay costs.
